This is an exciting time to join the team, The post will be based remote working at the moment due to COVID - 19.

The role and responsibilities
-Provide support and advocacy to enable clients in the community or residing in accommodation to obtain services on a day-to-day basis; e.g. health, housing, legal advice.
-Conduct assessments and develop support plans covering the housing, employment and social needs, specific to each client, which will assist them to integrate back into their families and communities.
